What this concern includes
Includes recurring deficiencies or hazards involving people working in restricted or inadequately separated spaces adjacent to, beneath or between moving or potentially moving heavy vehicles or machinery, where vehicle or machinery movement can cause fatal crushing or struck-by injury and dedicated separation, immobilisation or safe-position controls are inadequate.
