What this concern includes
Includes failures in the bounded primary-care mental-health interface that leave primary-care clinicians without appropriate ongoing involvement, follow-up, communication or awareness of changing mood, disengagement, risk or care needs after mental-health concerns or transfer of care; include failures by care coordinators or related services to maintain the information and contact needed for primary-care oversight.

- Excludes generic primary-care workload, continuity or communication deficiencies where no changing mental-health need or mental-health care interface is identified.
- Excludes failures confined to a named mental-health pathway, such as crisis-team access, CPA coordination or post-discharge follow-up, when that pathway supplies the more specific supported boundary.
