Includes failures in paediatric processes for recognising, assessing or escalating respiratory distress, including atypical presentations in children with myopathies, respiratory-rate assessment, oxygen-saturation monitoring and recognition of respiratory red flags such as shortness of breath, chest pain or cyanosis.
