Includes failures of governance and accountability controls specifically dedicated to multidisciplinary team decisions, including safeguards against inappropriate unilateral override, clear allocation of decision ownership and leadership, defined thresholds for MDT consideration or escalation, and mechanisms to review or challenge departures from MDT decisions.
