What this concern includes
Includes failures in hospital or mental-health-service arrangements for compiling, accessing, communicating, updating or handing over clinically and operationally relevant information to police when a patient is absent, missing or reported AWOL, including information needed for police enquiries, risk assessment, location and return arrangements.

- Excludes the broader missing-person response, including search, classification or location failures, when the information-sharing process with police is not itself deficient.
- Excludes generic police or healthcare information-sharing failures without an absent, missing or AWOL mental-health-patient context.
