Includes failures to interpret behaviour as a possible communication of distress, unmet need or increased risk, including normalising persistently unusual behaviour, misclassifying distress-related behaviour as obstruction or misconduct, and failing to trigger further assessment or protective action when behaviour changes or remains unusual.
