Rebecca Leanne Shaw · Prevention of Future Deaths report

On 22/12/15 I opened an inquest into the death of Rebecca Leanne Shaw who, at the date of her death was aged 32 years. The inquest was resumed and concluded on 27/1/17
I found that the cause of death to be: -
1a. Multiple skull fractures
1b. Severe head injury

I arrived at a conclusion of road traffic collision

At approximately 11.15am 12/12/15, ████████ was the driver of a Honda motor scooter with the deceased Rebecca Leanne Shaw riding pillion. Whilst ████████ made a right hand turn at the entrance of Soi Khuan Yang Charoen to cross a four lane dual carriageway known as Phra Barami Road, Patong Sub District, Kagthuy District, Phuket Province, Thailand. It was then that ████████ motor scooter was struck by an oncoming Toyota Vigo pick up truck driven by ████████. As a result Rebecca was thrown from the motor scooter and when taken to Pa Tong hospital she was found to have died from the injuries she sustained.

During the inquest I heard evidence to suggest that ████████ view of the oncoming traffic to her left was obstructed by a road sign and trees, itself on a bend, before commenced her right turn from the entrance of Soi Khuan Yang Charoen. I also heard that the lay out of the road had no central reservation deep enough to full accommodate a vehicle
