While MACE, MARAC and MAPPA processes were all engaged, there was limited coordination between these forums and no single mechanism that retained oversight of the cumulative and escalating risk presented to Natasha. Viewed against the statutory expectations operating at the time, the principal issue therefore does not appear to be the absence of safeguarding structures, but rather the effectiveness with which those structures were connected.”
