Investigation and inquest
On 4th January 2013, I commenced an investigation into the death of Mr William Reid. The investigation concluded at the inquest on 26th August 2014. The cause of death was 1(a) bronchopneumonia; (b) [████████]. The deceased had been dead for some days. The verdict was one of natural causes.
Circumstances of the death
On the 24th July 2013 at his home address [████████], the deceased was found deceased. He was aged 74 years. He had lived alone. There was a delay in the discovery of his death. He was admitted to the Cumberland Infirmary that day, where he was diagnosed with a urinary tract infection. He was discharged the following day. During the course of the inquest it became apparent that his admission to hospital in 2013 was not known to his GP.
Coroner’s concerns
His admission to hospital in 2013 was not known to his GP. The deceased was discharged from hospital without his GP being informed. There was a delay in the recognition of his deteriorating condition and in his subsequent admission to hospital. The risk is that future deaths will occur unless action is taken.
