Philip David EVANSON · Prevention of Future Deaths report

On 6 June 2016 I commenced an investigation into the death of Philip David EVANSON (age 55).

The investigation concluded at the end of the inquest on 12 October 2016.

The conclusion at the inquest was Road Traffic Collision.

The medical cause of death was 1(a) Multiple injuries.

At approximately 9.30 pm on 27 May 2016 Mr Evanson was riding his motorcycle on the A49 Tarporley Road, Tarporley, Cheshire when he collided with a car. He sustained multiple injuries during the collision which caused his death at the scene.

The road markings on the A49 Tarporley Road, Tarporley Cheshire at its junction with what is known locally as the ‘old’ Tarporley Road are significantly worn and in parts indistinct.

The markings in question relate to:

1. A ‘ghost’ island;
2. The lane dividing lines on the approach to the island;
3. Right turn arrows.

████████ of Cheshire Police gave evidence to this effect at the inquest. Photographs of the scene were produced to me in evidence. The officer indicated that the police had raised concerns with the relevant authorities immediately following the incident on a Police Form 43 (F43).
