At the time of the incident, the home’s telephone system was configured so that calls to the care home’s main line were directed only to the reception phone unless the caller selected option 3 (‘for Nurses’). If no one was present at the reception desk—such as on a Sunday afternoon, the call would not be answered immediately, which is what occurred in this case.”
